description:
The Louisiana Department of Health's Office of Aging and Adult Services is committed to improving its Community Choices Waiver program to provide better care and better health for more than 17,000 individuals awaiting services. You can help! Join us at one of several upcoming meetings to share your thoughts.

The Community Choices Waiver (CCW) is one of the most widely-requested Medicaid services in Louisiana, serving seniors and individuals ages 21 and older with adult-onset disabilities. CCW provides these individuals with a wide array of services including support coordination, nursing and skilled therapy assessments, in-home monitoring services, personal care, home modifications, home-delivered meals, and caregiver respite. Through CCW services, our goal is to provide an alternative to nursing home care, allowing these individuals to maintain their independence while continuing to live in the community.

Currently, the average wait time to receive services for these 17,000-plus individuals is more than eight years. That's something we want to change, and that is where you come in. We would like to hear your thoughts on how we can reduce this wait list and develop strategies to swiftly help people receive the care they need.

Individuals eligible for CCW services, family caregivers, community supporters and more are welcome. All meetings are open to the public. For more information, email tara.leblanc@la.gov. You can also learn more about CCW at http://ldh.la.gov/assets/docs/OAAS/publications/CCW Fact Sheet.pdf.
